Marin County HIV/AIDS CARE Council
Transition Work Group
The Transition Work Group will operate as a joint work group between the San Francisco HIV Health Services Planning Council (HHSPC) and the San Francisco HIV Prevention Planning Council (HPPC). The mission of the Transition Work Group is to prepare for and define the scope of work of the Joint Executive/Steering Committee that will mark the initiation of the merge of councils. The purpose of the merge is to ensure a continuum of HIV services for community members at risk for and living with HIV.
